Please fill out this registration form to register to attend Samantha’s “In-Person Group Meet and Greet.”

Once you register, you will also be placed into Samantha’s database.

Please make sure to add the Registration form to the cart. “Add to Cart” and then you will be taken to the form itself where you will add in your information.

There is a $75 processing fee to register.

Samantha’s Table reserves the right to reject any person who registers if it is discovered that the information provided is falsified or misleading in anyway.

Please note that your registration form will NOT be complete until you have emailed us photos AND answered ALL the questions below. 

For your photo submission, please send at least 2 head shots and 2 full body shots that have been taken WITHIN the past 3 months.

You DO NOT have to have professional shots taken.

Samantha’s Table makes introductions based on the criteria and requests of it’s paying clients and as a result, there is no guarantee that by registering here, an introduction will be made. Additionally, there is no guarantee that just because you are registering for a specific Matchmaking alert that you will be contacted for a date.
